Could you please supply with me sample letteer?

Dear friends, I'd like to file NIW by myself, but I don't have sample recommadation letter. I am doing biomedical research and have two 1st author papers and 4 second author papers, 7 presentation, 1 invited talk, 1 fellowship grant.
