1.you can go to any consulate to apply for your visa
2.as a canadian PR, you will get multiple entries visa within the validate H1 duration, for example, if you have two years left with your current H-1, you will get multi-entry visa for two year
3. how long you can get your visa depends on the consulate.
toronto: next day; ottawa, motreal, calgary, same day;Vancouver, not sure. it should be listed on US consulate at Vancouver, check it out
