I think the extra recommentations are very important. Get someone, not in your feild, but considered as experts, to write the letters. Take my case as an example. I had only three papers, no too many citations could be found. But my I-140 was approved without RFE. One thing I did was have OUTSIDE recommendations, in which, they indicated very clearly that their judement was from independent view. Independant reviewers can explain every thing. BTW, I had no NIH recommendation, but if you can get, it will greatly increase your chance. Good luck.
