Buy DIY pakage. Just cahnge your name your qulaification. Don't expect the official to look for evidences and get the conclusion the you are qualified for some of the requirements. You should say you are qulaified for these requrements, and provide the evidence. Then, the official look the evidence, and agree or disagree with what you claimed. Cover letter is very importent. Better be careful with it.
