just add the following words at the end of the document:

it is hereby to certify that i am competent to translate chinese to english and vice versa. i translated the above document from chinese to english. the english translation, to the best of my knowledge, is an accurate and complete one of its chinese version.

then ask the person who translated it (can be anyone, e.g.your co-worker) to sign and date it.
