140 part 4, item 6:
Has any immigrant visa petition filed by or on behalf of you?

Yes-attach an explanation.

1. what to Explain? Explain the reason the petion was filed? My previous company filed a 140 and got approved. But I left the company. How do I explain?

2.Also, at the end of the part 4, it says "If you answered yes to any of these questions, please provide the case number, office location, date of decision and disposition of the decision on a separate sheet(s) of paper."
what does "disposition of the decision" mean?
