3) Check to USCIS for $165.
4) Copy of USCIS receipt for I-485 (application for permanent residence) unless Form I-131 is submitted at same time as I-485.
5) Copy of identification page from passport.
6) Applications made after submission of the I-485 must include a copy of the I-485 receipt, and are sent to the same address to which you sent the I-485.
