回複：先謝.問:I-131 check list

1) Form I-131, Parts 1, 2, 3, and 7 (Check item “d” in Part 2 of the Form. In part 7 state “Adjustment applicant” and check the box for “more than one trip.” No additional documentation is needed. Download Form I-131 from www.uscis.gov, click on “Forms and E-filing.”
2) 2 color photographs -instructions at http://uscis.gov/graphics/publicaffairs/newsrels/04_08_02photo_release.pdf
3) Check to USCIS for $165.

4) Copy of USCIS receipt for I-485 (application for permanent residence) unless Form I-131 is submitted at same time as I-485.

5) Copy of identification page from passport.

6) Applications made after submission of the I-485 must include a copy of the I-485 receipt, and are sent to the same address to which you sent the I-485.
