One of my friends was diagnosed TB during the Physical Exame for Green Card application. He just followed what ever the treatment the immigration-Dorctor asked him to do---you know the TB treatment is free both here and in China. At the same time he still went ahead filed his I-140/I-485 the end of December in 2003 at Nebraska Service Center. And in April 2005 he got his GreenCard.
So, my oppinion:
1. Don't be afraid, follow the Doctor and finish all the Physical Exam and file your document ASAP;
2. Or you still feel not confident, why not just don't say any thing to the Doctor. You will be diagnosed or treated anyway if you had TB even now.
