2. Medical exam report I-693 and supplement (sealed);

3. Evidence of financial support:
(a) employment verification letter from Department of xxxx, University of xxxx, on business letterhead;
(b) tax returns with W-2 forms;

4. Original I-140 Petition;

5. G-325 A form

6. Copy of passport and visas;

7. Copy of I-94;

8. Copy of I-797;

9. Copy of birth certificate;

10. Marriage certificate.
