叩問: Employment letter or I-864

Working to file NIW before 10-1. Really unsure about these:
(1). Do I need to submit an employment letter?
(2). Do I need to submit Affidavit of Support (I-864)?
It seems from 485 instruction, we don't need to submit any of them. But some posts said we need to. Confused.
(3). Can I file 140+485 without medical exam this time? I mean do it later?
