Please help: About I-485 fees

Should we write 1 check ($315) for each member of family or one check for the whole family? How much is Biometrics Fee? (I'm sorry I could find it) and do we need to send it out with the I-485 application? For Form I-134, is it a must to be notarized? I'm sorry for so many questions. Thank you very much for your help!
