documents for EAD renewal

Hello, everyone, I will renew my EAD (my hu*****and's NIW based,I-140 approved,I-485 pending,). First EAD will expire end of Dec, 2005. My questions:

1: What kind of documents/forms I need to fill out and submit?

2: Where should I submit my application? ( I'm in Vermont Service Center)
