Qs filling forms status adjust

1. In G-325A, file # is not Alien Registration # and I have A # of OPT and should not put in as file #, right?
2. In I-765, one should choose "Yes" for "Have you ever applied Employ Auth." if using OPT before, right?
3. In I-131 for AP, the class of Admission should be the visa type of returning USA for the trip in future, e.g, H-1B, H-4, and not the visa type of latest entering US, right?
4. All forms I-485, I-131, I-765 for one person can be put in a single envelop to USCIS office with three payment checks attaching with each form separately, right?
5. Hou*****and and wife can not send all their forms in a single envelop, instead they should use two independent envelops for their own forms, right?
