travel to the Netherlands

Anybody had the experience in applying for a visa to travel to the Netherlands? I need to go there for business reasons this September. Would you please help answer the following questions regarding the visa:

1) Do I need to apply for the visa in person in a Consulate of the Netherlands in the US, or I can do it by mail?

2) If I need to do it in person, do I need an appointment?

3) What documents would I need to prepare (I am holding a US H1B)?

4) Do I need to apply the visa way in advance (e.g., 1 month or 2 months before the entry data)
