1. Form I-485
2. Two checks payable to “USCIS. ($315 for I-485 fee and $70 for fingerprint fee)
3. Two color photographs for I-485
4. Form G-325A
5. Report of medical examination
6. Evidence of valid nonimmigrant status:
7. Photocopies of passport identification, validity, visa pages and I-94s
8. Photocopy of I-140 receipt
9. Current employment letter for principal applicant, certifying continuing employment
10. Birth certificate (photocopy)
11. Marriage license (photocopy) of principal applicant and spouse
