國內醫學學位

I received my bachelor degree of medicine（醫學學士）, master degree of medicine（醫學碩士）and doctor degree of medicine（醫學博士）from a Chinese medical university. Now I am planning to apply GC through NIW and OR. Should my degree be translated to M.D. & Ph.D or just M.D? Also can I just use the notarial certificate from China (with English translation), or should I do the evaluation? I appreciate your kind suggestion. Thanks.
