1. I had same situation, I filed 765 for my wife but not for myself, I checked them all

Chose the code of the area in which you are stronger or currently working on

2.Your 485 is filed with 140, your LG's is filed with yours (thers is a word like family derveriate or something?? I forgot)

3. not sure, but I think RA is a part of your student time, not an employee, right? I would like to take accouny of your LG's in OPT

4. I don't know this form, I didn't file it

5. I did not file these
