OPT --h4

Hello friends, I need your help with H4 application.

My hu*****and is holding F1 OPT now and will be expired on Oct1, 2005, and my H1 is just be approved. Can we apply H4 ourselves? What forms should we fill in? Does the employer be involved ( must these forms be filed in the employer's name)? And do we need to file in fast speed way?
