求助I485 填表問題

本人正在申請485，但是我以前是F1， 是在美國國內換的H1簽證，H1後就沒有回去過。請問如何填寫 1> place of last entry into the united states? 是入關的地方嗎？還是批準H1的地方？我上次去了TORONTO， I-94上就表明TOR？什麽意思？Toronto 不是美國城市呀？

2> Nonimmigrant visa number? 是F1 visa 還是H1？但是H1 沒有VISA NUMBER 呀？我是在美國換的身份，沒有回去過。
