About the information on this 進入許可:
本進入許可必須附貼於據有相關附港簽注的有效往來港澳通行證上，方為有效。
May I understand that the 有效往來港澳通行證 is passport? Since she was in USA, usually, she should hold passport as the pass to travel oversee as well as to HK.
Do you think if this image is visa or 進入許可.

1.no, 有效往來港澳通行證 is not passport. it needs 戶口和身份證, as you said. It seems your daughter can't get it.
2. Like others suggested, contact 香港人民入境事務處.They either issue another visa, or allow her enter as is. Her case is very minor. don't worry. She is an adult now.
3. why didn't you disclose the original document at the 1st time? Thus everyone can understand your situation easily.
