Well defined hypoechogenic masses in the carotid space bilaterally with the dominant ones as follows:
*Right mid: 26.3 x 8.0 x 4.3 mm and 17.1 x 6.1 x 1.5 mm
*Left mid: 12.9 x 4.8 x 5.7 mm and 15.6 x 6.2 x 2.5 mm

THYROID SONOGRAM

*Right: Normal size at 5.1 x 1.4 x 1.7 cm with homogeneous echotexture. 16.8 x 14.2 x 14.7 mm well defined isoechogenic mass in the lower pole with thin hypoechogenic halo. Color Doppler interrogation demonstrates central and peripheral flow. Posterior to the lower pole is a 6.7 x 4.3 x 6.0 mm hypoechogenic mass.
11.1 x 5.0 x 8.0 mm well defined hypo and isoechogenic mass in the posterior interpolar region demonstrating peripheral flow with color Doppler Imaging.

*Left: Normal size at 4.8 x 1.3 x 1.4 cm with homogeneous echotexture.

*Isthmus: Normal thickness at 1.6 mm

INTERPRETATION

1. Normal appearing bilateral carotid space lymph nodes of questionable significance.

2. Dominant solid mass in the lower pole of the right thyroid lobe with some benign features. Differential diagnosis includes follicular adenoma and less likely possibility of malignancy

3. Small solid mass posterior to the lower pole of the right thyroid lobe could represent normal parathyroid gland or a small parathyroid adenoma.

RECOMMENDATION

You may wish to correlate the right thyroid lobe mass with a radioisotope scan.

You can never tell if a thyroid nodule is benign or malignant by the looks. You need to have a needle biopsy to get some tissues out and see. This is a very simple, safe procedure; you can find an endocrinologist to do it. The radiologist is not quite sure about the significance of an enlarged lymph node in your neck, depending on the position; it could be enlarged due to inflammation or other things.
