請教8老師，填寫ETA-750B表填寫的幾個問題

請問您幾個問題

1.填寫的第一項1. Name of Alien中有一個Maiden name和Middle name是不填寫還是寫NA，Maiden name好像是女士才填寫的。

 

2.填寫的第7項 Address in the United States Where you will Reside. 我現在在國內，不在美國，我也不知道將來會去哪裏，這一條怎麽寫？

3. 填寫的第8項 Name of Address of Prospective Employer of alien has job offer in U.S. 這一條是不是也可以不填寫

4.最後麻煩問您，這個表格是打印出來填寫內容嗎？
