謝謝8老師昨天的回答。我還有幾個問題請教8老師：

1. 填表的時候，我5年內工作單位隻有一個，那表上的工作單位2，3是不填，還是每欄都填n/a？
-Leave the others blank because you have indicated your work from what date to what date in the employer which covers the period of 5 years.

2. 同樣，part 4. information about your residence，隻有一個，其他的2，3，4是不填，還是沒欄填n/a？mailing address 和居住地一樣，是不填還是填n/a？如果填n/a，c/o那欄不讓填“/“？
-Same as above.

3. 表格必須用大寫字母填寫嗎？
-Not need unless you fill it in handwriting.

4. 交材料的時候需要準備cover letter 嗎？
-Not required.
