The part of I-94 included or not is very informative.
I agree the part of confusion can be ignored. I think it could also be due to some standardized form of contents which is not so applicable to me.
So I will go to apply for H1B visa next.
A follow-up Q: emplyment verification letter=>is the offer letter (with title, pay, term,...)from the department head and the Dean with my signature for acceptance OK? Or do I need to ask HR to produce a separate letter?
-The offer letter is fine, but it should be issued by the HR. You may ask HR to issue a letter to you for the H-1B application and they should know this matter well because I am sure you are not the only one their prospective employee which needs H-1B visa to enter US to work for them.
The offer letter is a copy as the original letter was signed and returned to the department head and Dean.
