1. 請教一個260表格中填寫的電話，應該填寫誰的

Provide the following information on all of your employers from the last ten years, starting with the most recent employer.

需要填寫的包括工作地點，Telephone Number,Job Title , Supervisor's Surnames ,Supervisor's Given Names 。

請問這裏的Telephone Number應該填寫自己的，還是雇主的？或者雇傭單位的?
-It is the employer's phone number.

2.Do you have documentation to establish that you have received vaccinations in accordance with U.S. law?

這個問題我應該選yes嗎？最後要求我們體檢時會有一個報告吧
