請教關於Certification by Translator的問題

在官方網找到的Certification by Translator，這個模板針對的是一個文件翻譯的證明。現在有很多中文文件做了翻譯，我是不是在the documents attached entitled 後做一個list。同時相應的，每個中文翻譯文件的最開始加一個"Title:the translation of XXX  "這樣的語句，比如一個專利說明書翻譯“Title:the translation of the specification of patent:XXXX（專利名稱）"，這樣和這個Certification中的the documents attached entitled能夠對應起來。
請教，多謝。

Certification by Translator

I [typed name], certify that I am fluent (conversant) in the English and Chinese languages, and that the above/attached documents are  accurate translations of the documents attached entitled 1.XXX   2. XXX  3.XXX  ......
