file a government complaint

Go to your state government website and find information about how to file complaint against auto insurance company.  Then submit complaint with evidence (policy report in your case) and your request of resolution.  Typically they must comply, or they are risking their business license.  Keep a copy of all documents in case you need to file a lawsuit.
