Speak like a marathon runner, not like 100 yard rush

I went the "physical" route first & the mental secondary. But really it was the physical change that made most definite mental changes, both consciously & subconsciously.

Try to look at it from a cause & effect viewpoint. What happens to you physically when you block/stutter? If you are like I was there was a lot of tension in my neck, mouth & lips. What is it like when you are alone and speaking fluently?

How could you speak in a manner without tension? A good start may be to speak your first word "soft" - this will help to keep your tension low. Think only of the first word - this will help to keep you from thinking about potential feared words in the rest of the sentence.

People who stutter normally begin speaking like a 100 yard Sprint athlete starts a race, lots of tension that can explode into blocks. You want to speak more like a marathon runner starts a race, relaxed, focused, at ease physically - not tense.
