485 RFE 請教高手：I-693 supplemental form

8月5號的第一個RFE要求做TB test，但沒有要I-693 supplemental form. 原文如下：

“Your orginal From I-693 cannot be accepted by this office because completion of your tuberculosis testing was not properly done; therefore, the form was not properly documented. You should return to the civil surgeion who first examined you (or another civil surgeon authorized by this Service) to properly complete the Form I-693 medical examination to include the required tuberculin skin test.

律師同時寄出我和我LP的reponse letters with TB test results provided後，9月2號USCIS收到LP的RFE response,她的case重新啟動，而我的case仍然是on hold, waiting for response.

更奇怪的事來了，最近我和LP又收到8月27號發出（9月9日才寄出）的第二個RFE，還是要求做TB test。原文除了上麵的以外，還有：

...see above...skin test. Please note that we cannnot accept copies of this form. You must submit a new I-693 and the I-693 supplemental form. Both forms must be completed originals and have original signatures from the applicant and the civil surgeon.

這次還要求I-693 supplemental form. 不知有沒有人用過這個表？是這個嗎？

上麵留有civil surgeon簽名的地方，但沒有applicant簽名的地方. 還是我找錯表了？

另外，既然我LP的case已經重新啟動，她的第二個RFE還要回嗎？
