Back door Roth 的1040-x表的問題請教

我和LD做了2021 back door Roth IRA conversion. 但忘記交8606表了，現在要交1040x和補上8606表。所有1040表裏的數字都不要改，就是在“Explaination of Changes"裏說明一下，然後附上8606表。請大家幫我看看我這個explanation說對了嗎？謝謝先.

Both xxx(husband's name) and (wife's name)made $6,000 after-tax traditional IRA contributions and converted them to Roth IRAs for 2021. However, Form 8606 were forgotten to be included in their 2021 tax returns. Both 1040-x and 8606 forms have now been submitted.
