再請大家幫看一下，我的情況用什麽eviction notice/form？

租客兩月未交房租，breaking lease （lease明年4月到期）已搬走。約好明天在出租房見麵。房子在亞特蘭大。

1. 我打算明天當麵給她eviction form/notice, 有人拍照。但用什麽eviction notice/form？3 day notice? and/or

2. 我當麵口頭通知她如果3天內不交，找公司追討所欠房租, late fee, rest of rental payment till lease ends， and leagal fee associated. will report it to credit credit bureau-請問怎麽操作？

3. 我知道她老公最近剛找到Google工作，可以找他公司要錢嗎？

我今天問她新家地址，她卻說用出租房地址，mail會forward to her new home address.
