Your child writes a heart-felt Thank-you letter/card,

 

expressing gratitude for the teacher's time and effort, reminiscing the time as a student, what was learned from the teacher, etc..

A gift with a monetary value tends to be construed as a price you placed on the teacher's goodwill.

Unless the amount is substantial, a card/letter would surffice.
