I-864, p.5, Question 21: do I *HAVE TO* have my wife fill out a separate I-864, or is this optional? (My answer to Question 20, Annual Household Income, includes both my wife's income and mine). 

-If you used your spouse's income to sponsor your father, then you file I-864 and your spouse files I-864A.
