8老師,能幫忙關於父親綠卡notice of intent to deny? short version

I have difficulty to paste, this is a short version of question and hope you can help.

2015-01-02-notice of intent to deny for my dad case.
 

The letter said :”
You claimed you last entry was on 09/07/2013. Submit proof
that you were inspected and/or admitted to the United States (Copy of your U.S
visa, passport stamp page, and/or I-94). “
 

We have the above materials and I  submitted  in
Feb 2014, the material is the same as my mom’s whose case just got approved. I do not know what they mean
“you were inspected”.  
 

Can you please advise me what I should do? Thank you so much!
