sub-floor sagging, moisture, inproper installation

I was told that if the sub-floor is plywood, tiles should not be laid on top of it directly.  Plywood, OSB are flexible.  A stiff cement board should be laid down first, then tile can be laid on top of cement board.

If the floor is sagging, you should feel it.
