Which form we need to file for a mistake on 1040 form

We just found one error on our 2011 1040 form. My hu*****and typed total income number wrong, it is only $18 less than W2. We are doing refinance now. The lender asks us to correct the error with IRS.

1. Which form can we use to file the correction?

2. How long will be taken to get IRS notice for the correction?
