your Dr. billed wrong, you were pregnant, he can't bill
you for "infertility", it's his fault. Send a email/fax to your Dr./insurance company explain your situation, and ask him to correct the bill to your insurance company. Keep a copy of the email/fax in case you need it in the future. That should be enough.
If your Dr. bother you again, file a complain (I don't know where, but there must be some kind of orgnizations.)
Wrong coding is a common mistake. Your Dr. should bill you under E&M code or consultation.
