單身老貓: Please help

In New York State, for a family court case, do I have to file to the court the consent to change attorney form indicating the out-going attorney and in-coming attorney. I don't have an in-coming attorney. I fired my attorney and I want to proceed pro se. Your help will be highly appreciated.
