What type of LLC did you choose when you filed your LLC?

If you chose "Sole Proprietorship", scheudle C is fine.
If you chose "Partnership", you have to file 1065 in December (not April).

After you requested the EIN, IRS should send you a letter indicates which IRS forms you must file for your LLC.
