legal advice please, regarding child custody change

My EX and I both agreed to change the custody of our son. In order to save money, we would like to do without an attorney.

I called the county court and was told that I need the following paper:

Motion to modify parental rights and responsibilities

Memorandum in support of the motion

Agreed entry



I went to library and looked up the Baldwin's law book and only find motion to modify parental rights and responsibilities, and could not find the other two.

Would anyone tell me what should the other two documents look like, some sample forms would be realy helpful. Please QQH if possible.

We don't want to write anybody's fault, it is just I have a stable family and it is better for my son. I also do not need my EX to pay child support if he does not want to pay.
