My daughter just had her name legally changed. Here is what we went through:

Go to your county court, register at the clerk's desk, pay the fee (around $100). The clerk will assign you a case number, a time, and court room number.

On the assigned day (morning or afternoon), come to the assigned court. Wait for your turn, the judge will ask questions for about 5 minutes. Once this is done, you will get a certified judgement indicating your name has been legally changed.
