Their parent will be 擔保 by them when they stay in USA or even 5 years GC period.

If the hospital can't get money from anywhere - I mean from goverment or insurance, they will send bill to his parent. Soon or later, will end up from collection to the 擔保 party.

The same as anyone 擔保 a student or tenant or business. The debt will fall to the 擔保 party.
