To STP: OPT to H1

hi, stp:

I came to usa in year 2000 as a student, and last Feb I changed my visa to H1B.(I was using my OPT in Jan 03, but worked for the same company.)

Shall I file my tax return as a Resident Alien or Non-Resident Alien, or Dual Status?

Since I was F1 in Jan 03, can I apply the treaty to get my first $5000 exempted for tax?

If I need to file as Resident, and if I can apply the treaty, where in 1040 I should put that $5000?
