Question for STP

hello, STP:

When I report interets on my retur,
(1)do I need to attcah the 1099 from the bank,
but that copy is for my record not for IRS,
(2)our university tax psecialist told me I don't need fill 8233, because I have attached 1042s. is that correct? Many Thanks.
