Resident Alien Question

This is my fifth year (start from Aug.2004) in US. I'm in H-1 status from July 2001.

My salary was subjected Federal, State and Social Security tax for last year (Jan. 2003 to Dec.2003).

Question: which form do I need to file for Tax? NR form or RA form? Thanks a lot.
