牙醫不能提供證據，保險公司拒付， 如何應對？

保險公司回答：The information recieved was reviewed by our consultant. There was not sufficient

documentation of disesea or injury to consider this service. Therefore, no benefits are payable

......

已經要求牙醫再提供更多文件。他不理睬。沒有保險公司這幾百美元，我還要欠他錢。他不停來信催款。

就拔一顆牙，收了1500美刀。太黑了。
