Can it be that easy? Risk is ~0 if for new construction

new construction, or the house owned clear by previous owner for many years.  If someone comes by and says the house is theirs, how can they prove it?  Or a bill not paid, why didn't they go after the last so many years until now.  Of course anything can happen...
