No. It is letter of Affidavit by your friend or your relative to show the acknowledgement of the relationship. It is not 864A, but I don't know much about it too. I thought it was a letter. but it seems NYC Marriage-based Green Card (sorry cannot type Chinese here) stated above that is a form.
