再次請教8老師

8老師好，我為父親同時遞交I130,I485,所以麻煩再問幾個問題：

1）. I-130，Part5，我兩年前已為母親申請了130，所以question1是 Yes.  

Provide name,place,date of filling: 這裏是不是填母親的姓名，我的Physical Address ？date filled 是不是母親I130的 received date (或 proirity date) ？

2）. I485's Evidence of continuously maintaining Lawful status since arrival in US:

Instructions says:  Include evidence for every time you entered the United States and for the time periods spent in the United States.

我爸從2011至今入境了5次，這樣我需要把過去每一次的記錄都要附上？ 還是隻用most recent I-94就可以？
