1.  What is your USCIS Online Account Number?

You can find your USCIS Online Account Number by signing in to your account and going to your profile page.

I just create the account for this GC renewal, I can't finid it in the online account. is it right to check "I don't have one"?

2. Where did you apply for your immigrant visa or adjustment of status? 

3. Where was your immigrant visa or adjustment of status issued?

I applied for mom 11 years ago and I didn't keep any record to answer these two questions. I guess the answers for the two question shall be the same, right?  However I don't know how to get the accurate information for these questions. Can I leave them blank? will it affect the renewal time if I don't fill in?

Will it be worse If I fill in with not accurate information from my memory ?
